Complex upper extremity injuries: targeted muscle reinnervation, free functional muscle transfer, and vascularized composite allotransplantation

چکیده

Restoration of upper extremity function poses a unique surgical challenge. With considerations ranging from ensuring appropriate skeletal support and musculotendinous ligamentous anatomy, restoring adequate vascularity innervation, providing sufficient soft tissue coverage, injuries present diverse range reconstructive problems. Recent history has been marked by an expansion novel techniques for addressing these complex issues. Sophisticated modalities, such as targeted muscle reinnervation, free functional transfer, vascularized composite allotransplantation, have become some the most powerful tools in armamentarium surgeon. This review article aims to define distinguishing features each modalities reviews their advantages limitations.

Upper Extremity Composite Tissue Allotransplantation Imaging

OBJECTIVE Upper extremity (UE) transplantation is the most commonly performed composite tissue allotransplantation worldwide. However, there is a lack of imaging standards for pre- and posttransplant evaluation. This study highlights the protocols and findings of UE allotransplantation toward standardization and implementation for clinical trials. METHODS Multimodality imaging protocols for a...

Targeted muscle reinnervation in the initial management of traumatic upper extremity amputation injury.

Targeted muscle reinnervation (TMR) was initially designed to provide cortical control of upper limb prostheses through a series of novel nerve transfers. Early experience has suggested that TMR may also inhibit symptomatic neuroma formation. We present the first report of TMR performed at the time of a traumatic shoulder disarticulation. The procedure was done to prevent painful neuroma pain a...

Facial Vascularized Composite Tissue Allotransplantation

Reconstruction of severe composite defects of the face is challenging for reconstructive surgeons. Despite several reconstructive techniques, the functional and aesthetic results of these patients are not acceptable. As a major organ, face, has an utmost importance in daily life. The specific characteristics of face require not a reconstruction, but a replace with same tissue. With the evolutio...

Functional neuro-vascularized muscle transfer for oncological reconstruction of extremity sarcoma.

The strategy of limb salvage following surgical resection of skeletal tumor has led to an increased demand for more complex reconstructive options in order to achieve better functional outcomes. Functional neuro-vascularized muscle transfer (FMT) is a beneficial tool for restoring joint movement involving the reconstruction of "movement" in the affected extremity. Until now, however, the clinic...

Targeted muscle reinnervation and advanced prosthetic arms.

Targeted muscle reinnervation (TMR) is a surgical procedure used to improve the control of upper limb prostheses. Residual nerves from the amputated limb are transferred to reinnervate new muscle targets that have otherwise lost their function. These reinnervated muscles then serve as biological amplifiers of the amputated nerve motor signals, allowing for more intuitive control of advanced pro...
